Health Navigation

With the encouragement of our health care partners, we developed our Community Health Navigator Program.
The purpose of the program is to increase health equity and access to a range of health services, to address disparities as well as health issues related to trauma.

  • The Client Health Navigator (CHN) meets first with clients referred to CORE El Centro for services.  

  • Their role is to assist the client in learning more about wellness and prevention, doing a whole person wellness assessment. The CHN makes a recommendation for the initial appointment and follow-up with clients, regarding the whole-person wellness plan.   

  • The CHN:

    • Educates clients as to the importance of follow-up care with traditional healthcare providers

    • Reinforces disease-specific education

    • Provides services to address barriers to health and social service resources   

  • The program provides safe spaces for our clients outside of Core’s space by referring them to agencies who will help them in their healing journey and keep them safe. 

CHNs are also a bridge between CORE El Centro and area partners. Natural healing and wellness services with well in tandem with behavioral and medical treatment, so we have built a large network of partners in Milwaukee who help identify individuals and families who might need our services and refer clients to our CHNs.
